(1) Fill out the Registration Form and Consent Form (see REGISTER NOW at bottom of page).
(2) Pay your $100 deposit fee by Pay Pal, Credit CardBY MAIL
Print and mail registration form to Deborah (Raven) Kelly at PO BOX 269 Alexander NC 28701. Include a check payable to WNC Heart. Be sure to include the name of the registered person!!!!!DEADLINES
DEPOSIT is due January 29th.
BALANCE is due by February 10th made payable to Idayanti Sudiro and mailed to Deborah (Raven) Kelly at PO BOX 269 Alexander NC 28701. Since Deposit and Balance due dates are so close together, you may mail in your balance payment at the same time you make your deposit.INTERNATIONAL Alumni may pay all fees in US cash funds on arrival at retreat. However, you still need to fill out all the required forms and submit them. You just don’t need to pay until you come to the retreat.
